Job description Job responsibilities MAIN ROLE AND PURPOSE OF THE POST The Assistant CareCoordinator will work across the Primary Care Network, under the direction ofthe Care Coordinator. They will utilisepopulation health intelligence to identify patients and support their care. SPECIFICRESPONSIBILITIES Data Build and utilisesearches within SystmOne to support PCN projects. Research health intelligenceto support the PCN strategic decision making. Refine clinical codingto support PCN projects. Carry out regularclinical audits. Monitor and report PCNachievement of PCN DES areas and IIF indicators. Monitor targetedinterventions to improve uptake or national cancer screening programmes. Monitor uptake of allimmunisation programmes. Patient Engagement Using invitation,education and encouragement, support patients to: Manage their long termconditions. Engage with cancerscreening programmes. Protect themselves withimmunisation programmes. Manage their ownhealth. Support patients tonavigate the health system. Support The Assistant Care Coordinatorwill support clinicians by: Producing data tosupport ongoing projects. Collecting and sharinginformation via Teamnet. SupportMulti-disciplinary meetings. GENERAL RESPONSIBILITIES Confidentiality You shall comply with the General DataProtection Regulations when processing personal data for patients and staff.UPDATEFC You must be aware of the Caldicott Principlesas they affect your work. You canaddress any questions in relation to this to your line manager. Any data breaches should be reportedimmediately to the Data Protection Officer. Allenquiries from the media must be referred to a Senior Manager. Healthand Safety All employees have a statutory duty of care for the safetyof themselves and others. Employees arerequired to report any hazards as soon as they are identified. All staff must contribute to themanagement of risk. This requires themto assess the risk in all situations, take appropriate actions to mitigate anyrisks identified and report all hazards, incidents and near misses. All buildingsoccupied by staff of TWG are non-smoking environments. No-one may smoke or vape in thebuilding. Staff who smoke or vapeoutside the buildings should cover their uniform. Have an excellent understanding of clinical coding. Have an understanding of medical terminology. Qualifications Essential Have GCSEs including Maths and English. Desirable Have undertaken or be prepared to undertake training in personalised care planning. Have undertaken or be prepared to undertake training in shared decision making.
