Advanced Nurse Practitioner / Advanced Clinical Practitioner (ANP/ACP)
Hours per week: Part‑time – approximately 18 hours.
Salary: £55,629 to £62,615 per full‑time equivalent (pro‑rata for part‑time).
Contract type: Permanent, subject to a probationary period.

Role Summary
This role offers an exciting opportunity for an advanced clinical practitioner with prescribing skills to join one of our GP practice teams. You will support core general‑practice activity within a flexible, collaborative environment, adapting your caseload to the practice’s needs. Typical duties include supporting residents in care‑home settings, managing long‑term conditions, and delivering urgent care and minor illness management. Your clinical expertise will focus on safe, evidence‑based decision‑making and patient‑centred care.
Key Duties & Responsibilities
Clinical Practice
* Work as part of a multidisciplinary team, delivering urgent and ‘on‑the‑day’ care, and providing long‑term condition clinics.
* Clinically examine and assess patients, making onward referrals when appropriate.
* Adopt self‑management and shared decision‑making approaches, supporting patients to achieve treatment goals.
* Prioritize health problems in complex or emergency situations, including initiating effective emergency care.
* Promote healthy lifestyles through motivational interviewing and ‘making every contact count’ techniques.
* Contribute to public health campaigns and vaccination programmes for adults and children.
* Maintain accurate and contemporaneous records in line with legislation and practice policies.
* Deliver high‑quality care grounded in NICE guidelines and continually evaluate its effectiveness through self and peer review.
* Participate in quality governance, audit, and learning events to improve service delivery and address inequalities.
* Build relationships with care‑homes, the PCN clinical team, and other community services, working collaboratively to meet cohort group needs.
Leadership & Professional Development
* Act as a clinical leader, ensuring patient needs are prioritized and that care delivery meets organisational standards.
* Model the organisation’s values and vision, acting as an ambassador across Surrey Heath GP practices and the PCN.
* Drive innovation and service improvement, evaluating new processes and technologies for impact on patient care.
* Develop and mentor staff, providing education, training, and support to junior professionals and community nurses.
* Contribute to the development of local guidelines, protocols, and an integrated frailty pathway.
* Manage audit and project work, planning workload and contributing to continuous quality improvement.
* Maintain professional registration, comply with regulatory standards, and engage in continuing professional development.
Clinical & Professional Responsibilities
* Build strong working relationships across the PCN, local practice and community services.
* Operate autonomously within professional and clinical guidelines.
* Delegate safely, supervising and supporting junior staff and students.
* Plan and administer your workload, including training sessions for the team.
* Participate in supervision, development, and appraisal processes, and maintain a personal development plan.
* Undertake any duties commensurate with your grade, ensuring compliance with organisational and statutory requirements.
Qualifications & Skills
* Advanced clinical practitioner or Nurse Practitioner registration (ANP/ACP) with prescribing authority.
* Experience in urgent care and minor illness management, preferably within a GP or primary‑care setting.
* Proficiency in managing long‑term conditions such as asthma, COPD, and diabetes.
* Strong clinical assessment, decision‑making, and patient communication skills.
* Knowledge of NICE guidelines and evidence‑based practice.
* Ability to lead, mentor, and contribute to continuous service improvement.
