General Practitioner - Solihull Locality Hub
Anexciting opportunity has arisen for an experienced and innovative GP to jointhe Solihull Locality Hub, a unique model of care delivered in partnership withUniversity Hospitals Birmingham (UHB). Youllbe part of a small, collaborative team focused on system-wide impact andintegrated care. This is not traditional general practiceit's your chance tolead change and deliver measurable improvement for the local population.
We are seeking an experienced and forward-thinking General Practitioner (GP) to join the Solihull Locality Hub a collaborative and innovative care delivery model designed to transform the health outcomes of the local population. This is a unique opportunity for a GP who thrives in dynamic environments and is committed to delivering system-level impact.

Main duties of the job
The successful candidate will work at the forefront of integrated care alongside a range of partners including University Hospitals Birmingham (UHB), contributing to a new way of working that is distinct from traditional general practice. You will play a pivotal role in shaping care pathways, supporting complex patients, and driving measurable improvements across Solihull.
Key Responsibilities:
* Provide high-quality, person-centred clinical care to patients in the locality hub setting.
* Work autonomously and at the top of your license while also contributing as part of a close-knit multi-professional team.
* Manage clinical complexity and diagnostic uncertainty with confidence, using a breadth of generalist knowledge.
* Engage collaboratively with secondary care partners, community services, social care, and other stakeholders.
* Contribute to the development and implementation of innovative models of care tailored to local population health needs.
* Identify system gaps and opportunities, and co-design solutions that improve outcomes and reduce unwarranted variation.
* Use data-driven approaches to demonstrate population health impact and inform continuous service improvement
We are ideally looking for someone available on Monday (morning and afternoon), Tuesday (morning and afternoon, and Thursday afternoons. However, as a flexible employer, we welcome applications from candidates who may have alternative availability.

* Management of acutely unwell and deteriorating patients.
* Familiarity with latest best practice, e.g. NICE Guideline NG245
* Strong understanding of population health principles and a desire to influence wider health system outcomes.
* Excellent communication and teamwork skills, with a proactive and flexible approach.
* Confidence in working across organisational boundaries and adapting to non-traditional care models.
* Experience working in integrated care, urgent care, or multidisciplinary teams.
* Experience in managing patients within care home facilities.
* Interest or background in quality improvement, data analysis, or digital health.
* Evidence of leadership in service transformation or pathway redesign.
* Familiarity with the Solihull locality and its health challenges.
