Highly Specialist Cardiac Physiologist (Cardiac Rhythm Management)

To perform clinical physiology duties within the sphere of ‘Cardiac Rhythm Management’, acting as a physiologist/clinical scientist in this area. The post holder will provide specialist professional direction and expertise in the technical environment of the device implantation and follow‑up as well as other aspects of CRM. They will act as a resource within the department to provide expert and specialist advice.

The post holder will be responsible for maintaining and developing standards of performance and competency within cardiac rhythm management.

The post holder will continuously monitor day‑to‑day service delivery, and suggest/recommend actions for improvement to the Head of Invasive Cardiac Physiology.

Please note, only applicants with experience of working independently in CRM clinics and CRM labs with accreditation will be shortlisted.


Main duties of the job

Post holder to perform the duties of a highly specialist independent practitioner, to competently perform a wide range of complex and highly complex specialised cardiac investigations, both invasive and non‑invasive; including electrophysiology studies, complex device implants and follow‑up; analysis, interpretation and reporting of results.

Specific areas include, but are not restricted to:

* Electrophysiology (diagnostic & therapeutic)
* Implantation and follow‑up of Implantable Cardioverter
* Defibrillators (inc. S‑ICD and EV‑ICD)
* Implantation and follow‑up of Pacemakers including Conduction Tissue Pacing, leadless and wireless devices
* Implantation and follow‑up of Cardiac Re‑synchronisation Therapy devices
* Implantation and follow‑up of Implanted Cardiac Monitors
* Follow‑up of remote / home monitoring devices

To ensure appropriate and thorough documentation of all procedures on the departmental IT system, (PaceNet and EPIC) to record all aspects of the service.

To act as an expert clinical role model and resource to staff in order to optimise quality of care delivery through facilitation of evidence‑based/best practice care.

To work clinically up to 90% of their total working hours as the service requires. Clinical work includes on‑site working across the trust estate as well as home working. The post includes a commitment to a non‑residential on call.


About us

These are exciting times in the rhythm management service at Guy's & St. Thomas'. We have one of the most dynamic training programmes at both undergraduate and post graduate levels including the higher specialist scientific programme (HSST). We implant a wide range of devices including leadless and wireless system, large CSP pacing cohort, epicardial pacing and EP for a range of arrhythmia including an established VT programme. We can offer the opportunity to develop skills in our paediatric service at Evelina Children's Hospital on the St. Thomas' site.


Job responsibilities

The post holder will be required to keep their professional, specialist scientific and clinical knowledge and skills up to date and to engage in and promote evidence‑based practice within their field. They will be fully aware of, and work within, their professional scope of practice and within the standards of conduct, performance and ethics for their professional and regulatory bodies. The post holder will lead audit and practice reviews within their area of expertise. They will be expected to act as a role model and mentor to trainees and to junior colleagues.

The post holder will be responsible for the evaluation and introduction of specialist scientific and technical knowledge and equipment within the cardiac rhythm management area. They will provide scientific expertise to other health professionals including its analysis and interpretation within the clinical decision‑making process. They will provide complex scientific information in an accessible manner to non‑experts. They will be responsible for the monitoring and assurance of the scientific quality of their area and will develop and implement processes to demonstrate and maintain its quality. The post holder will require IT knowledge and skills commensurate with their role as a healthcare scientist specialist within their field.


Person Specification


Knowledge/Qualifications

* BSc in cardiac physiology or relevant alternative
* Professional Cardiac CRM Exams (BHRS/IBHRE/EHRA)
* Hold an ILS or ALS certificate
* Evidence of post graduate study in clinical or management area
* Higher training qualification (e.g. Cert Ed, FAETC)
* Professional registration with HCPC as a Clinical Scientist


Skills

* Able to perform solo checks on a wide range of CRM devices (PPM, ICD, CRT, CSP, Leadless, Sub-Q)
* Able to support as solo physiologist in a wide range of CRM device implants (PPM, ICD, CRT, CSP, Leadless, Sub-Q)
* Able to communicate effectively at all levels in a multi‑professional environment.
* Excellent written and oral communication skills.
* Knowledge of cardiac science across a broad range of subjects at a level which allows data interpretation and provision of an expert scientific opinion to contribute to patient management.
* Able to support solo in theatres for epicadial cases


Experience

* Works solo in CRM Lab for at least 1‑2 years (+/- listing device types implanted PPM, ICD, CRT, CSP, Leadless, Sub‑Q)
* Works solo in CRM clinic for at least 1‑2 years (+/- listing device types followed up PPM, ICD, CRT, CSP, Leadless, Sub‑Q)
* Works solo in EP Lab for at least 1‑2 years
* Experience of working solo in extractions or epicardial implants
* Experience of staff management
* Experience of providing training & Assessment
* Experience of stock management
* Candidate a solo ILR implanter and or other equivalent CRM advanced practice
* Experience of on call for emergency CRM cases
