In the heart of Buckinghamshire countryside the Trust is located conveniently close to London and Oxford. We provide services to three main hospital sites, Wycombe, Amersham and Stoke Mandeville and some local GP practices. As a department we are ambitious and continually look to be at the forefront of innovation in Pharmacy careers. We are committed to the development of our staff.
We currently see 2-3000 patients a year in this pharmacist led Anticoagulant Clinic initiating and reviewing oral anticoagulation with the DOACs. In addition we run warfarin dosing clinics in 3 GP practices in South Bucks.
We are looking for an enthusiastic, dynamic and motivated Pharmacist to join our pharmacy led prescribing team working in clinics both in the acute Trust and in GP practices to optimise prescribing of anticoagulants and assisting with training to both primary and secondary care providers.

Education & Qualifications
Essential criteria
10. Registered Pharmacist: Member of General Pharmaceutical Council
11. Independent Prescriber
Desirable criteria
12. Post-graduate certificate in clinically based pharmacy or recognised equivalent
13. Education qualification
Experience
Essential criteria
14. Very strong clinical experience of long term conditions at Post Registration Pharmacist level
15. Delivery of pharmacy services to a range of specialities
16. Delivery of a pharmacy service to a specialist level
17. Experience of project management
Desirable criteria
18. Evidence of significant relevant Continuing Professional Development
19. Experience of training of other pharmacy staff
20. Experience in pre-registration training

22. Work, communicate and provide effective leadership within a team
23. Clear and concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers and to ensure records are kept accurately and in a legible manner
