The post holder will be responsible for a defined caseload under the direction of the Lead Stoma CNS. Providing specialised expert clinical practice, advice and psychological support for patients who are undergoing stoma formation in Colorectal and Peritoneal Malignancy services and their ongoing stoma pathways. The post holder will provide and contribute to the development of the specialty service and provide support and training for carers and health professionals across Hampshire Hospitals Foundation Trust. As member of the Stoma Clinical Nurse Specialist team, you will continue to develop strong relationships with your patients and their carers, offering high level care and expert advice to support them through their surgery and recovery. You will have a key role within the patient pathway, providing specialist input into the MDTs and supporting members of the MDT with new patient referrals when stoma formation is discussed as a surgical option or your expertise in managing recurrent advanced Pseudomyxoma Peritonei. You will be expected to utilise your skills and knowledge to provide expert advice to local and national stoma teams to enhance collaborative working an ensure seamless service for patients.
Main duties of the job
· Assess and treat own caseload of patients, managing the pathway from referral onwards
· Deliver education within the organisation and to external agencies as required
· Liaise with local referral centres to maintain the patient pathway
· To development and review of stoma information literature across both Colorectal and Peritoneal Malignancy services.
· Develop the post holders’ network of peer support, nationally and internationally, to support best patient care
· Review stoma relevant SOP’s, policies and procedures
· Undertake nurse-led telephone, face to face assessments for patients throughout their stoma care pathway.
· Inform G.P and community services of any changes to patients’ stoma treatment plans
· Lead on highly complex patient assessment, and lead on stoma/fistulae management.
· To act as a member of local MDT for Colorectal and Peritoneal Malignancy Services.
