Job overview We are looking for an experienced and compassionate Dementia Crisis Practitioner to temporarily join our frailty pathway team. You will work closely with health, social care, and voluntary sector partners to deliver rapid, person-centred interventions, helping to prevent hospital admissions for people living with or suspected dementia. This role requires excellent clinical judgement, strong partnership working skills, and the ability to respond quickly and effectively to crisis situations. Main duties of the job • Complete rapid, holistic assessments for individuals in crisis. • Develop and deliver person-centred crisis intervention plans. • Work collaboratively with multidisciplinary and multi-agency teams. • Provide specialist advice and support to carers, families, and professionals. • Advocate for least restrictive, community-based alternatives to hospital admission. • Maintain accurate and timely clinical records and documentation. • Participate in safeguarding processes and multidisciplinary meetings. • Contribute to service improvement initiatives and audits. • Promote resilience, reflective practice, and wellbeing within the team. Check out our recruitment videos Recruitment - YouTube Detailed job description and main responsibilities Our team of dementia crisis practitioners provide specialised support to individuals with dementia who are experiencing acute behavioural or psychological distress. They work in a holistic system and are embedded within East Kent's frailty system. They have a focus on assessment and triage, crisis intervention, person centred approaches to manage behaviours that may challenge and ongoing education, support and signposting to system partners, families and carers. The programme's duration depends on role and can be from 2 days to 4 weeks (not always consecutive days). The Induction programme will provide an excellent opportunity for new staff to meet and engage with KMPT's subject specialists and include the required training. The programme may involve some travel to the various training venues across Kent and Medway, the main ones being Canterbury, Maidstone and Dartford.
