Main duties of the job
The post holder will be working as part ofthe wider Elmham Group of Practices Teams which include North Elmham, SwantonMorley and Toftwood Practices.
With support from the General Practitionersand existing Advanced Nurse Practitioners and Advanced Clinical Paramedic, thepost holder will be responsible for providing care of the presenting Patientthrough initial history taking, clinical assessment, investigations leading toappropriate diagnosis and treatment pathways. Demonstrate safe, clinicaldecision-making and acting within Professional boundaries expected. To workcollaboratively as part of the Multi-Disciplinary Team. Providing support tocolleagues within their direct team. Assisting with the development ofProtocols, Education, and training programmes, along with involvement inResearch and Audit Programmes where appropriate.

Elmham Groupof Practices (EGoP) is a GP Surgery in rural Mid-Norfolk serving a population of13,500 patients across three sites in Elmham, Swanton Morley and Toftwood. Ourmain site is set in beautiful surrounding countryside in North Elmham with bothother sites within comfortable proximity. The fine City of Norwich is easilyreached by car within 35 minutes.
EGoP has a team of 70+ healthcare professionals and administration support staff. All three sites offer awelcoming and friendly environment to work in and team spirit amongst surgerystaff is high. The 4 Partners, Management Team, Clinical Team, andAdministration Team communicate well between them.
Our fantastic multi-skilled ClinicalTeam consists of General Practitioners, Nurse Practitioners, Practice Nurses,and Health Care Assistants to carry out all the necessary duties to deliver acomprehensive General Practice service ensuring the highest standard of care toall our patients. We are part of the PCNfor Mid Norfolk and through our network we have a range of ARRS roles withinour practice to support us in delivering high-quality care to our patients. Thepractice employs its own Pharmacist and Visiting Nurses. Locations Elmham andSwanton Morley has an on-site Dispensary. We are proud to be a trainingpractice to GP Registrars and Nursing students.

Job Description
The post holder will beworking as part of the wider team serving the surgeries at North Elmham,Swanton Morley and Toftwood Medical Centre. With support from the Partners andother Nurse Practitioners, the post holder will provide care for the presentingpatient from the initial history taking, clinical assessment, diagnosis,treatment, and evaluation of their care. They will demonstrate safe, clinicaldecision-making ensuring that they always act within their professionalboundaries. They will work collaboratively with the multi-disciplinary generalpractice team to meet the needs of patients. The post holder will providesupport to the Clinical Manager on clinical audit, research, the development ofpractice clinical guidelines, procedures and protocols, education programmesand quality initiatives such as QOF and CQC (Care Quality Commission)inspections.
PersonSpecification
The successful candidatewill be:
- NMC(Nursing and Midwifery Council) (Nursing and Midwifery Council) Registered withcurrent and up to date Revalidation and Registration.
- QualifiedNurse Practitioner with an appropriate level Degree/Masters.
- Holda valid Non-Medical Prescriber qualification.
- Holda clean and valid Driving Licence.
- Haveexperience with Minor and Acute presenting illness with advanced level decisionmaking skills.
- Holdthe ability to work autonomously and have good decision-making skills.
- PrimaryCare experience is preferred but not essential.
- Knowledgeof working with System 1.
- Knowledgeof working to QOF (Quality Outcomes Framework) requirements.
- Abilityto follow protocol and procedural practice and National guidelines
- Excellentteam working and communication skills
- Flexibleand multiskilled with practice based and home visiting patient care andassessment.
DBS
The post would be subject tothe Rehabilitation of Offenders Act (Exceptions Order) 1975 and the provisionof a satisfactory DBS check.

- Referappropriately to the wider Clinical Team and Secondary Care where appropriate
