Key Responsibilities Clinical Care Act as a first point of contact for patients via triage, face-to-face, telephone, or online consultations. Independently assess, diagnose, treat, and manage patients with undifferentiated and complex presentations. Request and interpret diagnostic tests and investigations as appropriate. Prescribe and review medications in accordance with clinical guidelines and legal frameworks. Provide personalised, holistic care that promotes self-management, prevention, and health improvement. Identify and manage long-term conditions where appropriate, working closely with the practice nursing and GP teams. Refer patients to other services and agencies when clinically indicated. Maintain accurate, contemporaneous records in line with professional and practice standards. Leadership, Education & Quality Contribute to service development, clinical governance, and quality improvement initiatives within the practice. Support the clinical team through supervision, mentoring, and sharing of advanced clinical knowledge. Participate in audits, reviews, and data collection as required to support DES and QOF delivery. Keep up to date with national and local policies, guidelines, and clinical best practice. Maintain professional registration and comply with relevant codes of conduct (NMC, HCPC, or GMC).
