Job summary

We are expanding our clinical capacity and looking for a highly motivated and enthusiastic part time Practice Nurse to join our friendly dedicated team at Fountain Medical Centre.

Working as part of the Nursing team to deliver chronic disease management, childhood immunisations, cervical screening and travel vaccinations. We are looking for someone who has experience in Diabetes and has completed the Diabetic diploma.

The ideal candidate should have excellent people, planning and organisational skills and able to manage your own workload.

The successful candidate will work alongside a multidisciplinary team made up of GP's, Advanced Nurse Practitioners, Clinical Pharmacists and Nursing team to provide treatment and a high standard of patient care. Support, supervision and mentoring will be provided. You will receive an attractive package of competitive salary, commensurate with experience and qualifications, NHS Pension, allocated study leave in addition to 27 days annual leave (pro rata) plus statutory bank holidays.


Main duties of the job

Key duties will include:

* Undertake a range of chronic disease management appointments
* Undertake diabetes long term condition management
* Undertake respiratory long term condition management
* Undertake cytology screening
* Provide childhood immunisations and travel vaccinations
* Provide wound care and dressing management

Following agreed clinical protocols with referral to lead nurse or GPs as appropriate

Must be a Registered General Nurse and hold full registration with the NMC. Diabetic diploma required and other chronic disease diplomas are essential.

Hours are 25-30 per week

We are a dynamic, forward thinking training practice wishing to deliver locally innovative and integrated services to our practice population of approx 17500 patients. We are a SystmOne practice based in Morley, Leeds and our last CQC rating was good.

Our clinical team of GP's, ANP, practice employed Pharmacist and large nursing team work alongside an evolving team of network employed pharmacists, pharmacy technicians, first contact Physiotherapists, care co-ordinators/social prescribers which reduce our workload.

Job responsibilities

Clinical practice

* Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
* Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
* Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
* Work with patients in order to support adherence to prescribed treatments

* Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
* Anticipate barriers to communication and take action to improve communication
* Estimate and maintain effective communication with individuals and groups within the practice environment and with external stakeholders
* Act as an advocate when representing patients and colleagues

Delivering a quality service

* Recognise and work within own competence and professional code of conduct as regulated by the NMC
