Overview

Royal Berkshire NHS Foundation Trust (RBH) invites you to join our clinical or non-clinical teams during an exciting period of change. Following our CQC inspection, we have been rated ‘Good’, with improvements to the Trust’s overall rating. This position is based primarily at Townlands Memorial Hospital in Henley-on-Thames.

An exciting opportunity has arisen for a Patient Pathway Coordinator (PPC) to join our Clinical Administration Team in CAT 1 Department. The role involves providing support to clinical and non-clinical staff. You will be a point of contact for administrative issues relating to patients’ pathway of care, working as part of a multidisciplinary team, and providing expertise in the proactive management of the patient pathway from referral to discharge. This ensures the entire pathway is managed smoothly and high levels of patient and clinician satisfaction are achieved.

You will use Standard Operating Procedures (SOPs) to ensure that the function of the role is carried out correctly within given timescales. The CAT 1 Team is committed to providing colleagues with a positive experience and opportunities to build fulfilling and rewarding careers.


Responsibilities

* Act as a point of contact for administrative issues related to patients’ pathway of care, from referral to discharge.
* Provide proactive management of the patient pathway in a multidisciplinary team environment.
* Ensure high levels of patient and clinician satisfaction through accessible, customer-focused service.
* Apply Standard Operating Procedures to ensure consistent, timely delivery of role functions.
* Collaborate with clinical and non-clinical staff to support a smooth patient journey.


Qualifications and Skills

* Experience providing administrative support to clinical and/or non-clinical staff.
* Ability to act as a clear point of contact and work within a multidisciplinary team.
* Proactive, customer-focused approach and familiarity with SOPs.
