Job Summary
We currently have two exciting opportunities for enthusiastic Speech & Language Therapists to join our services: the Neuro Early Supported Discharge Team and the Community SLT Team.

The service supports acute hospital discharges and admission avoidance for patients following an acute stroke or other neurological event.
Community Speech and Language Therapy Service

Patients include those recovering from strokes, people living with Parkinson’s disease or dementia, and frail or older individuals who need support with communication or swallowing.

* Contribute to data collection where relevant as a member of the multidisciplinary team.
* NEURO ESD ROLE: Work as part of an interdisciplinary neurological community team, collaborating closely with other services/agencies such as acute stroke units, community rehab teams, virtual wards, and rapid response teams.

* Provide clinical support to other colleagues, including students, where appropriate.
* Contribute to client information, manage priorities, organise equipment/resources and liaise with other SLTs about continuing care following discharge.
* Be flexible in providing cover to community settings where most needed, as directed by the clinical services manager.
* Contribute, wherever possible, to the administrative processes that ensure the most efficient service delivery for clients.
