An opportunity has become available for a Nurse Associate at Primary Care Sheffield.
The Nursing Associate will work with the Nursing team and other members of the multi professional team, providing and monitoring the condition and health needs of patients within their care, contributing to the ongoing assessment, as agreed with supervising clinicians and in line with the PCS service pathways. You will be expected to support patients in the management of long-term conditions (Asthma, Diabetes and COPD), Cervical Cytology, Family planning, Wound Management, Travel Immunisations, and all other general treatment room duties
Main duties of the job
Duties And Responsibilities:
Provide continuing direct and indirect care to patients from across Sheffield.
Provide clinical observations. phlebotomy, flu vaccinations, ECGs, 24-hour ECG & BPs and venepuncture, Cervical Screening and other designated clinical tasks as required
Monitoring and support of the management of patients with long-term conditions / chronic diseases, using a holistic patient centred approach as per their own registered practices care plan.
Support patients to improve and maintain their mental and physical health and wellbeing.
Assist with the co-ordination of Infection Control policy and audit if requested.
Chaperone patients when required
Support the registered nurses to enable them to be able to focus on the more complex clinical care.
Contribute to the delivery of integrated care
Promote health and well-being to all patients, for example undertaking the NHS health check, Hypertension reviews, Prediabetes checks.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to colleagues and local community and voluntary sector services
Care for individuals with dementia, mental health conditions, and learning disabilities.

Clinical Responsibilities Individuals Care
The following list is indicative as tasks and responsibilities will vary depending on the care setting the Nursing Associate is working in.
Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management, and evaluation of care.
Support individuals with all aspects of care including daily living, providing person-centred care, and promoting health and independence through awareness raising and care navigation to other professionals as appropriate.
* Provide general and specific health screenings to the practice patients - within agreed protocols with referral to the wider MDT i.e. Practice Nurse, ACP, or GPs as necessary.
Perform and record clinical observations including blood pressure, temperature, respirations, pulse, weight, height, and glucose monitoring.
Undertake clinical tasks including cannulation, venepuncture, ECGs.
Demonstrate an ability to complete ongoing monitoring templates.
Administer vaccines after completion of relevant training under PSDs.
Undertake cytology once training and sign off has been completed.
Long term condition management, following care plans in place, under the supervision of a Registered Nurse.
Undertake home visits as needed for the patient population working within scope of practice.
Accurately record nutritional and fluid intake.
Ensure the privacy, dignity and safety of individuals is always maintained.
Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate.
Report back and share information with the registered nurses on the condition, behaviour, activity, and responses of individuals.
Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals.
Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being administered.
Assist in the implementation of appropriate action to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers
