Overview
We are currently seeking a dedicated and experienced Practice Nurse to join our busy Primary Care Network working across 4 practices. This is a new role within the PCN - working in a hub and practice-based model. As a Practice Nurse, you will be responsible for providing high-quality nursing care to our patients and working/supporting with the ARRS Roles in the PCN as well as the wider practice teams.
Main duties
* Familiarity with PCN and Primary Care ways of working is a strong advantage.
* Conducting health assessments, including physical examinations and health screening
* Providing health education and advice to patients on a range of topics, such as healthy living and disease prevention
* Administering vaccinations and other injections
* Taking blood samples and carrying out other diagnostic tests
* Supporting patients with long-term conditions, such as diabetes and asthma
* Supporting practices with the management of chronic diseases
* Cervical Smears
Requirements
* Registered Nurse with a valid Nursing and Midwifery Council (NMC) pin
* Minimum of 2 years' experience working as a Practice Nurse
* Experience in chronic disease management
* Experience in Cervical Smears
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team

About us
You will be joining a Primary Care Network of 4 member practices in the Hanley, Bucknall & Bentilee area with circa 35,000 patients.
Based out of the PCN Hub, you will be either visiting housebound patients, or working from one of the member practices in clinics.
We have a multi-disciplinary workforce within the PCN covering Social Prescribing, Pharmacy, Physiotherapy, Mental Health, Paramedics, and frailty.
Job responsibilities
Duties andResponsibilities
1. Clinical
2. To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems
3. Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
4. Provide travel health advice and administer appropriate vaccinations
5. Treat patients with minor injuries including wound care and dressings
6. Provide directly and assist GPs with contraceptive services
7. Handle sensitive conversations whilst communicating news to patients/parents/carers and provide appropriate ongoing support to meet identified needs
8. Act as chaperone when required
9. Undertake health promotion to help educate patients
10. Select, carry out and interpret results of specific tests
Administration
11. To maintain accurate patient records and enter onto the computer using agreed Read Codes/Snomed codes
12. Ensure accurate completion of all necessary documentation associated with patient health care
13. To collect data and participate in regular PCN audit and research projects
14. To contribute to the preparation of any PCN development plans
General
15. To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
16. To maintain a safe and therapeutic environment for all PCN staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to
17. To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the PCN
18. To maintain own continuous professional development and ensure PDP and revalidation are kept up-to-date in line with the requirements of the PCN
19. Attend and participate in staff and PCN meetings and in-house clinical training sessions
20. Any other reasonable duties that may be required from time-to-time

Qualifications
* RGN Qualification
* Current NMC registration
* Accredited training in chronic disease management
* Accredited training in anticoagulation management
* Cervical cytology qualification
