Generic Support Worker - Community Response Teams

We are looking for kind, compassionate, creative problem‑solvers with excellent communication skills to join our expanding team across the York and South Hambleton and Ryedale areas. In this role you will be a crucial part of the Multi‑Disciplinary Team, delivering high‑standard health care to patients in their own homes, facilitating short‑term rehabilitation, reablement and recovery to avoid unnecessary hospital admissions and enable patients to recover after an acute hospital stay.
You will work closely with patients and their families, nursing and therapy colleagues, and build connections with health and social care partners across the local system. Taking a re‑ablement approach, the service supports people to return to, or remain in, their own home.
We are looking for candidates who can motivate individuals, aid their recovery and maximise independence to achieve the goals that matter to them. Roles are available across York and South Hambleton & Ryedale areas.
Main duties of the job
You will undertake duties delegated by Nursing, Physiotherapy and Occupational Therapy teams, and be responsible for providing holistic, person‑centred, compassionate care to patients aged 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patient’s home.
We provide a 7‑day service, between the hours of 8 am – 8 pm. Successful candidates will take part in a rota that covers these hours, though we offer a flexible and varied shift pattern.
The ability to transport yourself and equipment across the locality during the course of a normal working day is required. Access to a vehicle that can be used for business purposes and a full, valid driving licence is required.

Qualifications
* A Levels (3) or Level 3 national certificate or Level 3 diploma, or Senior Healthcare support worker apprenticeship, or Mathematics and English to Level 2: Functional Skills/GCSE Grade C
* Proactively undertake relevant specialist or departmental training to achieve required knowledge and competency for the role
* Up‑to‑date personal portfolio of learning
* Evidence of transferable workplace training and competencies
