An exciting opportunity to join the Community mental health - Older adult service, an expanding team working with patients with both dementia and functional disorders.
The role of the Memory assessment clinician is open to CPNs and Occupational Therapists interested in developing their skills in Memory assessment work. The position will involve working predominantly with patients presenting with memory difficulties, completing assessments of cognition and supporting the diagnostic process. As the team grows and develops, it may also include work with patients presenting with functional illness.
Responsibilities
* Memory Assessments within clinic or patient home environment, dependent on patient need.
* Timely diagnosis, following close joint working with Consultant psychiatrists.
* Pharmacological treatment, as per guidance from Consultant psychiatrist, as appropriate depending on profession.
* Post-diagnosis support and signposting.
* Monitoring and reviewing of patients started on treatment.
* Contribution to duty system.
* Assessment of risks at home and support to address/mitigate utilising MDT approach.
