Are you passionate about supporting children and young people with complex needs? Do you thrive in a role that bridges health, education, and social care to create real change? Dorset Council is looking for a dedicated Keyworker – Family Worker to join our dynamic Preparation for Adulthood Team.
About the Role
As a Keyworker, you will provide personalised, community-based support to children and young people aged 0–25 with autism and/or learning disabilities who are at risk of mental health hospital admission or are currently in inpatient settings. Your role will be pivotal in:
* Building strong, trusting relationships with children, young people, and their families/carers.
* Acting as a key point of contact to coordinate care and support.
* Supporting the implementation of personalised plans.
* Working across services to remove barriers and improve outcomes.
* Participating in Care, Education, and Treatment Reviews (CETRs).
What You'll Do
* Provide direct support to children and young people on the Dynamic Support Register (DSR).
* Collaborate with multi-disciplinary teams across health, education, and social care.
* Contribute to service development and evaluation through data collection and feedback.
* Attend and contribute to national and regional Keyworker Community of Practice events.
Travel Requirement
This role involves significant travel across the Dorset Council area. A suitable vehicle must be available on most working days.
