Responsibilities
* Evaluates the clinical needs of the patient, including Elder and Private Services, Hospice and Certified; initiates, develops, implements and makes necessary revisions to the patient's plan of care; addresses all problems in the plan of care and documents accordingly.
* Annual outcomes meet National Benchmarks.
* Instructs and provides documented supervision of the Home Health Aide plan of care as required.
* Collaborates and communicates on a regular basis with the Team Leader, and members of the inter-disciplinary team as needed.
* Records narrative documentation to reflect care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
* Refers to community resources as appropriate.
* Notifies MD/other involved care providers of significant changes in the patient’s condition.
* Completes admission and other paperwork on a timely basis.
* Obtains and clarifies valid MD orders for all care delivered.
* Assures compliance with the Nurse Practice Act and Standards of Practice in the State of Massachusetts.
* Functions within the parameters of knowledge and experience, practicing safely and competently.
* Performs caregiver evaluations, updates plan of care, admits patient PC and HMK to services and provides skilled nursing services.
* Oversee the health care rendered to patients by conducting patient visits, supervisory visits while employees are on duty, and through telephone contact with patients and employees.
* Assumes off-hours on-call responsibilities as needed to assure that there is on-call coverage at all times.
* Maintains environmental safety for the patient. Accurately and safely utilizes all equipment and supplies appropriate and necessary for the care of patients.
* All other duties as assigned.
Qualifications
* Current RN/LPN license in Massachusetts.
* CPR certification required.
* Minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN.
* Home care and IV experience, computer skills and BSN preferred.

* Schedule Details: 22.5 hrs. per week, Mon-Fri, 8a-4p, No Weekends, No Holidays.
