As a Senior care worker, you will be visiting service users to complete assessments, reviews, draw up care plans, provide personal care, building relationships, and make a big difference to those who need it whilst gaining great job satisfaction.

Areas covered: BR1, BR2, BR4, BR5, BR6, BR7 (Bromley, Southborough, Bickley, West Wickham, St Mary Cray, Farnborough, Orpington, Chislehurst, and all surrounding areas).

Principal duties and responsibilities
* To act as a Senior Key worker and formulate care plans in accordance with the service users’ needs and wishes and to update as needs change. This includes ensuring that all junior care staff contribute to the best of their ability to the efficient running of the service and the creation of the right atmosphere.
* To supervise junior staff, providing support and assistance, providing them with PPE and documents as and when needed. This includes carrying out spot checks on Home Carers, completing reviews and assessments on Service users as and when required.
* To report any issues to the management team, complete and maintain records of daily activities, observations, and direct hours of service.
* To ensure service is delivered in accordance with all relevant policies, procedures.
* To ensure that the Service User has a full understanding of their Care and ensuring a Service User has a full understanding of the number of hours allocated to them per week and how the use of these hours is to be negotiated between the Service User and the Provider, including times of delivery, where specific times for medication purposes are to be prioritised.
* To support Service Users to achieve their defined outcomes as detailed in their Care Delivery Plan. These will include the personal activities of daily living such as personal care and communication and the instrumental activities of daily living such as meal preparation and mobility. It may also include support to Service Users to remove obstacles to community participation such as enabling the Service User to walk safely down their own path or steps which may be their first step to reintegration within the community.
* To proactively engage and liaise with other appropriate professionals or services to support a Service Users needs which could be facilitated through Adults’ Health and Care department (e.g., Telecare, Health Visitor, Dementia Advisors, Reablement, GPs, Health and Social Care Professionals, Meals on Wheels Services, Equipment Store, Housing Departments, Department of Work and Pensions, Citizen Advice Bureau etc).
* To act as community connectors using Connect to Support and by signposting and facilitating access to services that offer support with tasks such as shopping and prescription collection etc.
* To facilitate access to organisations that will support community integration and participation in activities that enhance the person’s overall well-being. To fully understand the importance of working in partnership with others i.e. understands why it is important to work in partnership with key stakeholders and professionals, advocates and others who are significant to the Service User.
* To be fully aware of their responsibilities in relation to Safeguarding, Protection from Abuse, and their professional boundaries.

Education and Qualifications
Essential:
* Good Basic Education Level 2 in Health and Social care or equivalent NCFE Level 2 in Safe Handling of Medicines.
Desirable:
* Level 3 in Health and Social care or equivalent.
General Intelligence

* Good general intelligence.
* Clear reasoning ability.

Work Experience
Essential:
* Ability to follow instructions.
* At least 6 months’ experience in a role caring for the elderly.
