St Ann's Medical Centre have an exciting opportunity for an up to 9 session (minimum 4 sessions) GP to joinour growing practice. We are a large, supportive and fun practice team within a PMS multi-professional training practice offering plenty of opportunity fordevelopment of individual skills and interests and a real focus on educationand training within the practice and involvement across the local area.

We have just over 19000 patients across three sites. Weare looking for a proactive and highly motivated person to become part of ourfriendly and supportive team which includes the Surgery's GPs, Business Manager, Advanced Clinical Practitioners, Practice Nurses, Pharmacist's, Paramedics, Reception team and Administrative staff.

The successful applicant will be encouraged to take anactive role in the leadership and development of the practice, incorporatingall the aspects of being a GP.

The individual should enjoy working as part of a cohesive team, possess good communication skills and have good general IT skills.

Flexibility, compassion and a "can do" approach isessential for this position.


Main duties of the job

* To deliver a highly effective medical care to the entitled population.
* To ensure that the provision of services commensurate with the GMS/PMS contract.
* Generic prescribing adhering to local and national guidance.
* Effective management of long-term conditions.
* Processing of administration in a timely manner, including referrals, repeat prescription requests, blood results and other associated administrative tasks.
* On a rotational basis, undertake telephone triage and duty doctor roles.
* Contribute to the successful implementation of continuous improvement and quality initiatives within the practice.
* Accept delegated responsibility for a specific area (or areas) or the QOF
* Work collaboratively, accepting an equal share of the practice workload
* Attend and contribute to weekly clinical meetings and other practice meetings (e.g. PLT)
* Support the partners in achieving strategic aims, including enhancing income and reducing expenditure of the practice

Job responsibilities

Purpose of Job

To deal witha wide range of health care needs in the primary care setting, providingmedical services to patients and such other duties as reasonable delegated toyou by the Practice, that are required of the Practice in providing servicesunder Personal Medical Service (PMS) Regulations.

Main Duties

In accordance with the Practicetimetable, as agreed, the post-holder will make themselves available toundertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking and signingrepeat prescriptions, processing of blood results, paperwork and correspondence in a timely fashion.

Making professional, autonomousdecisions in relation to presenting problems, including history taking,examination, investigation, whether self referred or referred from otherhealth care workers within the Practice.

Assessing the health care needs ofpatients with undifferentiated and undiagnosed problems.

Screening patients for disease riskfactors and early signs of illness

In consultation with patients and inline with current practice disease management protocols, developing care plansfor health.

Providing counselling and appropriatepreventative health care and advice.

Admitting or discharging patients toand from the Practice caseload and referring to other care providers asappropriate.

Recording clear and contemporaneousconsultation notes to agreed standards.

Compiling and issuing computergenerated acute and repeat prescriptions.

Prescribing in accordance with thePractice prescribing formulary (or generically) whenever this is clinicallyappropriate.

Providing reports eg Department ofwork and Pensions, medical certificates and death certificates.

Support the practice with supervisionof junior doctors/ nurse team.

Provide cover or buddy for absentcolleagues.

Electronic medical note keeping forall patient contacts, including template entries supporting clinical recall,QOF data and maintaining comprehensive medical histories (i.e. Deaneryrequirements to support the GP Training Practice Status of the Practice).

In general the post holder will beexpected to undertake all the normal duties and responsibilities associatedwith a General Medical Practitioner working within primary care.

Other Responsibilities within thePractice

Awareness of and compliance with allrelevant practice policies or guidelines ,eg,Prescribing, confidentiality, data protection, health and safety.

A commitment to lifelong learningand audit to ensure to ensure evidence based best practice.

Contributing to evaluation, auditand clinical standard setting within the Practice.

Attending training and events organisedby the Practice and other agencies, where appropriate.

Participation in an annual individualperformance review, including taking responsibility for maintaining a record ofown personal and or professional development.

Clinically related administrative andnon-clinical duties necessary for the delivery of Personal Medical Services.

General

To participate in an annualindividual performance review process where the job description will bereviewed, objectives will be agreed, performance monitored and personaldevelopment needs discussed.

To act responsibly in respect of yourown and colleagues health and safety and welfare, following safe workingpractices and complying with the Practices health and safety policy, thisincludes attending mandatory and safety training.

To be aware of and co-operate in theimplantation of arrangements for risk management.

The Practice has a statutory duty toensure that patients, clients and members of the public are consulted andinvolved in decisions about local health service developments. As an employeeyou should be mindful of this and do what you can, appropriate to your role andlevel of responsibility to meet this duty.

To comply with all other St AnnsMedical Centres policies and procedures.

Supporting the practice staff andresponding to requests for advice and assistance from the Practice reception,secretarial and nursing staff.

You will be required to give dueregard to the Quality and Outcomes Framework (QOF) and contribute to achievingmaximum points in all areas.

Freedom to Act

Professionally responsible for ownworkload, supervised by a partner operating within standard policies andprocedures.

Physical Effort

Occasional physical effort may berequired e.g. patient collapses or faints etc.

Working within a confined space,moving equipment, kneeling to give patient care.

Mental Effort

Frequent requirement forconcentration responding to predictable and unpredictable patterns.Concentration as dealing with patients with multiple long term conditions andfollowing assessment further problems may be highlighted.

Emotional Effort

Frequent exposure to distressing oremotional circumstances, eg delivering and receiving bad news, dealing withfamilies of patients used as a support mechanism, dealing with cancer andother terminal illnesses etc.

Possible exposure to highlydistressing or highly emotional situations, e.g., cardiac arrest, severe asthmaCOPD exacerbations fits etc.

Working Conditions

Occasionally dealing with abusive andaggressive patients.

Visiting patients in their home.

Apply infection control measureswithin the Practice according to local and National guidelines including:universal hygiene precautions, the collection and handling of laboratoryspecimens several times a day, swabs, segregation and disposal of wastematerials, decontamination of instruments and clinical equipment.

Dealing with the reporting andtreatment of sharps injuries.

Working Relationships

Internal working relationships to interface with all members of the Practice.

External working relationships To interface with Patients, Relatives, Carers, SocialServices, Police, Voluntary Services, Department of Work and Pensions,Secondary and Primary and Tertiary Care, Department of Education andProfessional Nursing Bodies.

Confidentiality

In thecourse of seeking treatment patients entrust the Practice with or allow thePractice to gather sensitive information in relation to their health or othermatters. They do so in confidence and have the right to expect that staff willrespect their privacy and act appropriately.

In theperformance of the duties outlined in this Job Description, the post holder mayhave access to confidential information relating to patients and their carers,practice staff and other healthcare workers. They may also have access toinformation relating to the Practice as a business organisation. All suchinformation from any source is to be regarded as strictly confidential.

Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the Practice may only be divulged to authorised persons inaccordance with the Practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data.

Summary

The post holder will support thePractice as required across the range of duties as appropriate within thegrading of this post. In the context of a rapid and ongoing change within theNHS and the Practice the above responsibilities represent the currentpriorities and requirements for the post. These priorities will develop andevolve over time. Any significant changes will be the subject of fullconsultation with the post holder.
