Clinical Practice: Conduct physical health assessments and screenings for patients with long-term mental health conditions, including cardiovascular risk assessments, diabetes checks, respiratory assessments, and metabolic monitoring. Undertake annual physical health checks for patients receiving shared care (e.g., antipsychotic monitoring in partnership on behalf of primary care). Interpret clinical findings and initiate appropriate treatment plans or referrals. Prescribe medications safely and appropriately, in line with local and national guidelines.
Manage and review physical health issues in collaboration with mental health and primary care teams. Promote lifestyle advice and health education (e.g., smoking cessation, diet, physical activity). Maintain accurate and up-to-date clinical documentation in line with local and national policies. Leadership and Service Development: Contribute to service planning and quality improvement initiatives focused on integrating physical and mental healthcare.
Support junior staff and contribute to the clinical supervision and training of colleagues where appropriate. Liaise effectively with GPs, community teams, pharmacy, and other healthcare professionals to ensure continuity of care. Governance and Professional Practice: Adhere to clinical governance frameworks including risk management, incident reporting, and audit. Maintain own professional development in line with NMC/HCPC registration and revalidation requirements.
Follow all local and national policies relevant to the ACP role, prescribing, and mental health services.
