As well as our inpatient unit at Woodside Avenue work within the community palliative care-East and West; our support to patients in their own homes in partnership with our specialist Palliative Care teams, providing emotional and practical support to patients, their families and carers. Social care work within community palliative care team provides psychosocial care to patients, families, and their carers pre & post bereavement. They act as advocates for people with advanced life limiting illness and their family / carers, aiming to enable individuals to make informed choices to manage their health, wellbeing, and lives independently for as long as possible.
