Children's Safeguarding Care Coordinator
We have a fantastic new job opportunity at Erewash PCN. We're hiring a Children's Safeguarding Care Coordinator.
In this role, you will coordinate care for vulnerable families, which will significantly reduce the admin workload for our practices and GPs. You'll also strengthen connections across our practices and improve our regulatory compliance.
As our Children's Safeguarding Care Coordinator, you will provide essential safeguarding support throughout all our practices. This means collaborating closely with clinicians, practice support staff, and external agencies. It's a diverse and challenging role, so you'll need excellent people skills and organisational abilities. You will manage sensitive and confidential information daily, so integrity is crucial.
Main duties of the job
Gatherinformation to inform the assessment of risk when a concern is raised by amember of staff or advised to the practice by a partner agency.
Sharechildren's safeguarding relevant information with other agencies in an agreedformat and timely manner.
Withthe approval of the Lead GP, and oversight by the responsible GP:
a)Partake in child case conferences.
b)Attend Strategy meetings
c)Provide Child protection Conference reports
Workalongside the practice manager and GP leads to:
Coordinate and quality assure regular practice children's safeguardingmeetings (Multi-Disciplinary Teams (MDTs).
Maintain and utilise children's safeguarding riskregisters.
Support the audit of practice children's safeguardingactivities.
Search clinical systems to identify children not broughtto appointments.
To improve data quality of recording of safeguardingconcerns in the patient record

About us
Erewash HealthPartnership (EHP) was established on 1 April 2018 and is a single GPPartnership comprising of eight constituent practices (see below). EHP employs approximately 250 people who workwithin the eight constituent practices and within the EHP central team, whichprovides support services e.g., finance, operational, data & performanceetc.
As well asproviding high quality core primary care services, EHP is the prime providerfor delivering services to Erewash Primary Care Network (EPCN), as well asoperating its own services for EHP Partners.

You will work as part of a small team linking into the wider General Practice Safeguarding lead GPs.

Forman important link between the practice and the Derbyshire Children'sSafeguarding Board, to:

b)Partake in local safeguarding projects and pilots.
c)Provide feedback to Derby & Derbyshire Integrated CareBoard (DDICB) teams on how best to support practices.
6.Reportto EHP Senior Leadership Team and working with the Safeguarding Lead to:
a)Support the audit of EHP children's safeguardingactivities.
b)Implement and assure EHP policies and procedures thatsafeguard patients.
c)Demonstrate a-b above to CQC when required.
Demonstrate a flexible attitude and beprepared to carry out other duties as may be reasonably required from time totime within the general character of the post or the level of responsibility ofthe role, ensuring that work is delivered in a timely and effective manner.
Work in accordance with the EHP, PCNpractices policies and procedures.
Contribute to the wider aims andobjectives of the PCN to improve and support primary care.
