As a Social Worker experienced in working in Intermediate Care settings and hospitals, you will be reaching out to those in crisis or who are acutely unwell within the community and the hospital itself.

The office is based in Accrington; there is an expectation to travel to different locations to provide assessments, therefore a driving licence and access to your own vehicle is essential. This is a 7‑day service (including bank holidays) with a requirement to work up to one in four weekends. We offer pay enhancements for weekend working.

Responsibilities

* Provide social work assessments and support for clients in Urgent Care and Short Term Support and in hospital/Intermediate Care settings.
* Engage with service users in crisis or acutely unwell conditions and promote independence and well‑being.
* Travel to different locations to perform assessments (driving licence and access to own vehicle required).
* Work within a multidisciplinary team to achieve the best outcomes for service users.
* Support and advocate for service users to access appropriate services and interventions.

What We Are Looking For

* Passion – commitment to helping others, fostering independence, and building trust through empathetic, approachable care; ability to form strong working relationships.
* Challenge – ability to manage a fluctuating workload, provide guidance to NHS colleagues and other professionals, and maintain service user‑centred practice across varied duties.
* Values driven – respect and compassion within a supportive, multidisciplinary team; willingness to work flexibly to achieve positive outcomes.

Benefits and Development

* A range of discounts and benefits from Vivup, access to support groups, public service pensions, cycle to work schemes, eye tests, and more.
* Support for development, supervision, mentoring, opportunities for progression and personal growth within the role.
* Inclusive culture that values diversity and flexibility, including home‑based options where appropriate.
