Clinical Practitioner – Acute Visiting Service

Excellent opportunity for a Registered Health Professional qualified in Physical Health Assessment and Non-Medical Prescribing to join our Acute Visiting Service (AVS). The role lies within the Acute Care at Home programme, providing care to patients in their homes to avoid hospital admission. Working hours: 13:00‑20:00 Monday‑Friday with extended coverage 08:00‑22:00 any day of the week.
Main duties
* Carry out assessment, diagnosis and treatment of patients referred to AVS.
* Provide clinical decision‑making and management plans in the home setting.
* Collaborate with the Acute Care at Home Service, Virtual Ward Team and 2‑hour Urgent Community Response.
* Support all aspects of Acute Care at Home, prioritising AVS referrals from primary care.
* Ensure safe, evidence‑based prescribing and medication management.
* Document care and maintain accurate patient records.
* Engage in continuous professional development and participate in audits and service improvement.
* Commence and lead multidisciplinary meetings and rounds when required.
Key responsibilities
* Recognise and work within own competence, following professional standards.
* Establish effective relationships with internal and external partners.
* Lead clinical rounds, huddles and MDT meetings.
* Triage new referrals and determine required level of support.
* Perform advanced physical examinations and deliver treatment plans.
* Prescribe, deprescribe and monitor medications.
* Lead complex case management.
* Perform clinical procedures such as catheterisation, venepuncture, cannulation and ECG.
* Communicate clearly with patients, carers and colleagues.
* Provide training and mentorship to junior staff.
* Produce accurate documentation and contribute to audits.
* Drive service development and promote policy implementation.
* Manage equipment safely and adapt to non‑purpose built rooms.
* Represent the service in departmental meetings.
Person specification
* Registered practitioner with current NMC/HPC registration.
* Qualifications: Physical Health Assessment, Non‑Medical Prescribing.
* Experience in autonomous practice within a community or acute setting.
* Substantial post‑registration experience, especially in emergency/acute/general medicine.
* Strong clinical reasoning, decision‑making and communication skills.
* Proactive in CPD, mentorship, and policy change.
* UK driving licence, vehicle access required.
