Overview
Community Mental Health Nurse Band 6 (South Warwickshire) – Coventry and Warwickshire Partnership NHS Trust (CWPT). This role is based in Stratford Upon Avon, with services spread across two bases in Stratford Upon Avon and Leamington Spa.
Responsibilities
* Provide the nursing voice as part of the multi-disciplinary team, implementing, evaluating and reviewing all aspects of the service user's care.
* Manage a caseload of service users and deliver interventions to meet each individual's needs.
* Day to day delivery of the service to individual clients and their families and carers via a range of evidence-based interventions.
* Medication management, new patient assessments and duty as required.
* Support the physical health and well-being of service users and enable access to services in a way which respects their values and supports their rights.
* Evaluate the effectiveness of interventions in relation to supporting recovery and well-being of service users.
* Opportunity to supervise junior staff, develop leadership skills and clinical skills depending on your chosen career path.
* Training and supervision provided; opportunities to attend the nurses forum to support your CPD.
* The team is looking at specialist roles to utilise staff skills and interests to the fullest.
Requirements / Qualifications
* Registered Mental Health Nurse (Band 6) with experience in adult Community Mental Health Teams.
* Ability to work within a multi-disciplinary team and deliver evidence-based interventions.
