About the role Tŷ Anwen is a residential mental health service in Caerphilly, supporting up to 16 individuals living with serious and enduring mental illness and complex needs, which may include self-harm. Our team works to a recovery-focused model of care, offering clear therapeutic boundaries and outcome-driven support that builds independence and confidence. As a Complex Care Project Worker, you will play a key role in empowering and supporting individuals on their recovery journey. Working as part of a dedicated team, you’ll help to develop, implement and review high-quality, person-centred care and support plans that reflect each individual’s needs, preferences and aspirations. What you’ll be doing: Supporting individuals to live independently within a care setting. Support, prompting and administering medication. Engagement in social activities Completing and updating support plans, risk assessments and risk management plans. Carrying out supervisions Completing assessments from referrals Be responsible for the service in the absence of the manager. Undertake relevant training for the role. Liaise with Social Services, Benefits Agencies, Housing department and other relevant agencies Organising group activities with the individuals at Ty Anwen or in the community Providing emotional support to the individuals Skills and Experience Needed Please ensure your application evidences experience of the following: I.T. literacy GCSE Grade C or above in Maths & English (or equivalent) Experience working as part of a team Minimum of 2 years’ experience working with individuals with mental health needs Values At Codi, our values are Compassionate, Authentic, and Progressive. Waking night rate: 20% Enhancement from 11pm to 7am Enhanced DBS which we will pay for Social Care Wales registration is fully reimbursed Pension Choices: Defined Contribution (DC) Scheme with matched contributions up to 10%. Or, Defined Benefit (DB) 1/80th of final salary for every year of service. 3 x Life Cover linked to pension membership Saving scheme with competitive interest rate Give As You Earn scheme for tax efficient charity donations From 1st April 2026 the option to be either weekly or monthly paid.
