About the role Tŷ Anwen is a residential mental health service in Caerphilly, supporting up to 16 individuals living with serious and enduring mental illness and complex needs, which may include self-harm. Our team works to a recovery-focused model of care, offering clear therapeutic boundaries and outcome-driven support that builds independence and confidence. As a Complex Care Project Worker, you will play a key role in empowering and supporting individuals on their recovery journey. Working as part of a dedicated team, you’ll help to develop, implement and review high-quality, person-centred care and support plans that reflect each individual’s needs, preferences and aspirations. What you’ll be doing: Supporting individuals to live independently within a care setting. Support, prompting and administering medication. Engagement in social activities Completing and updating support plans, risk assessments and risk management plans. Carrying out supervisions Completing assessments from referrals Be responsible for the service in the absence of the manager. Undertake relevant training for the role.
