Job Description: Triage and Prevention Assistant, Rochdale
We are currently looking for a dynamic and forward-thinking individual to join our team as a Triage and Prevention Assistant. This is an exciting opportunity to help shape the implementation of our Prevention & Digital Strategic plan, which aims to build digital inclusion across adult care. The team is responsible for providing Assistive Technology, including careline services, and exploring alternative methods to delay the need for care and support across Rochdale, enabling residents to live healthier for longer.
In this role, you will be part of the front door team for all new contacts into adult care, focusing on delaying and reducing care and support needs by supporting the delivery of assistive technology and careline services.
This position is within an expanding team that will be delivering and testing a range of existing and new initiatives. Your responsibilities will include working alongside other prevention team members to complete prevention assessments and to assess for and fit assistive technology.
