Job Summary
We are seeking an experienced and motivated Practice Nurse to join our friendly and supportive team at Riverside Medical Practice. Serving a growing patient population of over 11,500 patients, we are committed to delivering high‑quality, patient‑centred care within a modern and welcoming practice environment.
Key Responsibilities
* Provide comprehensive nursing services including independent management of chronic disease clinics (diabetes, asthma, COPD and hypertension).
* Carry out cervical cytology and childhood/adult immunisations, travel vaccinations.
* Deliver wound care, ECGs, NHS Health Checks and health promotion activities.
* Work collaboratively with GPs, practice nurse, healthcare assistant and multidisciplinary team.
* Maintain accurate, confidential clinical records using SystmOne.
* Participate in clinical audit, quality improvement and service development.
* Communicate effectively with patients, carers and external professionals.
Qualifications
* Registered Nurse with current NMC registration.
* Evidence of continuing professional development.
* Previous experience working as a Practice Nurse within General Practice.
* Independent management of chronic disease clinics (diabetes, asthma, COPD, hypertension).
* Experience of cervical cytology (smear tests).
* Experience delivering childhood and adult immunisations.
* Experience of health promotion and preventative healthcare.
* Experience maintaining accurate clinical records.
Desirable
* Experience using SystmOne.
* Experience of NHS Health Checks.
* Experience of travel vaccinations.
* Experience of family planning and contraceptive services.

Full‑time hours are preferred; part‑time applications will be considered for the right candidate.
