We are looking for a enthusiastic, motivated individual to workalongside our current Practice Nurse, GPs and administration staff. We are looking for either experienced practicenurses or nurse practitioners and will consider part or full time hours.
Experiencein chronic disease management, theQuality and Outcomes framework and other targets required of modern day generalpractice are essential skills required forcandidates. Applicantsshould also have experience of cervical cytology, family planning, spirometry,travel advice and childhood immunisations. Experience of minor illness,independent consultations and prescribing are essential skills for nurse practitioners. Applicants must be able to work as part of ateam as well as under their own initiative and have excellent organisationaland communication skills. Salary will be dependent onexperience and skills.
Main duties of the job
Chronic disease monitoring in line withrequirements of QOF

Routine immunisations/Childhood immunisations
Chaperoning and assisting patients whereappropriate who are being examined by another clinician

Following agreed clinical protocols with referralto senior nurses or GPs as appropriate
Other tasks willinclude
Chaperoning and assisting patients who are beingexamined by another clinician.
Undertake home visits for chronic diseasemonitoring and flu immunisation
Responsibility of actioning blood resultsrelevant to chronic disease monitoring

Weaver ValePractice is a friendly, well-established, enthusiastic teaching practice with9,200 patients. We have four partners and three salaried GPs. Based in a largepurpose built Health Centre, sharing with another general practice, we have anintegral pharmacy, district nurses and health visitors on site, and areadjacent to Halton General Hospital in Runcorn.
Job responsibilities

Maintain general tidiness and cleanliness of nursesand treatment rooms
Participation in administrative systems in thePractice
Attend and participate in any Practice meetingswhen required.
Undertake home visits for chronic diseasemonitoring and flu immunisation
Any other delegated duties appropriate to the post
Monitoring QOF and undertaking call and recall ofpatients for chronic disease clinics
Responsibility of actioning blood resultsrelevevant to chronic disease monitoring
Set up for andassist with:
Minor surgery procedures including joint injections
IUD insertion and removal
Implanon insertion and removal
The practice nurse will refer to asupervising member of the clinical team/GP if deviation from the protocol isrequired, if unsure in any way, or where clinical judgement is required beforeproceeding.
Drugs will be administered only on the basisof written and signed medical authorisation.
Record keeping and administration
Make accurate andcontemporaneous records of all patient encounters in accordance with practicerecords protocols and coding protocols
Accurately label anddocument patient samples and submit for collection and analysis.
Contribute to nursingteam record keeping (clinical, administrative, Health and Safety relatedrecords).
Maintain skills in usingpatient appointment system.
The post-holder will participate in any training programme implementedby the practice as part of this employment, such training to include:
Comply with NMC Code of Conduct and keepregistration up to date, documented and checked in the practice on an annualbasis.

Order most economic supplies available and use witheconomy.
Order and maintain stock of health informationleaflets and keep displays up to date.
Equipmentmaintenance and calibration
Contribute to care and maintenance of equipment.
Organise repair and regular maintenance andcalibration of equipment, including equipment maintenance/checking/calibrationlog.
