Job summary

An exciting opportunity has arisen within our Community Mental Health Service (CMHS), based in Bridgwater. We are seeking an enthusiastic and compassionate Practitioner (Registered Mental Health Nurse, qualified Non-Medical Prescriber) to join our well established, supportive and dedicated multidisciplinary team.

This role will support adults with serious mental illness (SMI) and complex needs, working across a shared caseload with a particular focus on delivering and developing our Depot, Clozapine and Wellbeing Clinics. You will be a key clinical leader within the team and will play an essential role in service delivery, medicines optimisation and patient-centred care.

This post is full-time (37.5 hours), typically 9am--5pm, with flexibility for condensed hours and varied start/finish times.


Main duties of the job

* Lead and deliver high-quality Clozapine, Depot and Wellbeing clinics, ensuring safe and effective monitoring, prescribing and follow-up care.
* Provide independent prescribing within your scope of competence, including initiation and ongoing review of psychotropic medication.
* Support timely access to medication, enhanced frequency of monitoring and medicines optimisation across CMHS caseloads.
* Contribute to CMHS assessments, offering immediate prescribing or medication advice where required.
* Provide clinical leadership, offering supervision, guidance and support to colleagues across the team.
* Collaborate with Consultant Psychiatrists, team managers and multi-disciplinary colleagues to ensure best practice and consistent service delivery.
* Participate in and promote clinical audit, governance, evidencebased practice and service development initiatives.
* Maintain accurate, confidential and timely patient records within the Trust's electronic systems.
* Work autonomously while accessing regular supervision from senior psychiatrists, NMP peers and clinical leaders.
* Engage in professional development, mandatory training and CPD relevant to your role in line with Trust expectations.

* Hold a defined caseload of individuals with severe and enduring mental health difficulties and complex needs.
* Provide specialist assessment, treatment planning and psycho-social interventions, including risk assessment and risk management.
* Deliver prescribing clinics--including Clozapine, Depot and independent NMP clinics--to support medication initiation, titration, review and monitoring.
* Offer expert advice on medication management to GPs, colleagues and partner agencies.
* Support smoking cessation initiatives and contribute to physical health monitoring as part of holistic care delivery.
