Job Summary
We are looking for a dynamic and flexible person to join our happy team in Soho, Central London.
Required skills and experience:
* Assess, implement, and evaluate treatment programmes that promote health and well‑being, including individual treatment plans for patients requiring longer term treatments such as wound care, asthma, COPD, diabetes, hypertension, cardiovascular disease, and mental health.
* Work with other healthcare professionals in monitoring and managing long‑term conditions, in line with national and local policies and practice needs.
* Review medication for therapeutic effectiveness, ensuring appropriateness to patient needs and evidence‑based practice.
* Support patients to adhere to prescribed treatments and medication reviews.
* Provide information and advice on prescribed and over‑the‑counter medication, including possible side‑effects and interactions.
* Support patients to adopt health promotion strategies, promote exercise, improve diet, and reduce risk‑taking activities such as smoking and excess alcohol intake.
* Support and manage health needs of women presenting for family planning and cervical cytology consultations.
* Recognise, assess and possibly refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
* Implement and participate in vaccination and immunisation programmes for both adults and children.
* Meet the needs of patients presenting for opportunistic wound care.
* Other practice nurse duties as required by the practice, including ensuring clinical practice is safe and effective, maintaining competence boundaries, and following protocols as a non‑medical prescriber when applicable.

* Routine immunisations and vaccinations
* Removal of sutures/clips
* Baby immunisations
* Health surveillance and general health advice
* Provide general and specific health screenings to practice patients, with referral to other members of the primary and secondary health care team as necessary.
* Chronic disease clinics (Diabetes, COPD, Asthma, Cardiovascular)
* New patient health checks
* Ensuring infection control measures are applied
Qualifications
* RCN accredited Practice Nurse (essential)
* Familiar with Systm 1 General Practice Software (desirable)
* Phlebotomy (desirable)
* Immunisations (desirable)
* ECG (desirable)
* Chronic Disease Management (desirable)
* Smear taking proficiency (desirable)
