CHAW PCN have an exciting opportunity for aPharmacy Technician to work across GP practicesto workacross our PCN which covers practicesacross Chelford, Handforth, Alderley Edge, Wilmslow (CHAW) Primary CareNetwork.
We are seeking an enthusiastic andforward-thinking pharmacy technician to join our PCN. The successful pharmacytechnician candidate will play an important role in complementing our clinicalpharmacists and other members of the PCN multi-disciplinary team. This is anevolving job role with huge scope for development as we expand our PCN team.
Main duties of the job
We expect the successful candidate to contribute to the delivery of a comprehensive primary care medicines management system acting on current best practice / service change requests from our clinical commissioning group. The Pharmacy Technician will be required to undertake activities predominantly at a practice level, but will need to be flexible to work to the wider demands of the network. For example, the role will involve liaising with community pharmacy teams to highlight and manage problems identified by community pharmacy, such as medicines waste.

Job responsibilities
As a Pharmacy Technician you will work as partof a pharmacy team and help implement our integrated model of prescribingsupport. The successful candidate will share our values and have the energy,skills and enthusiasm to contribute to shaping how we support our patients.
This post is part of the PCN rolesreimbursement scheme.
The main duties include the safe processing ofmedication requests, issuing prescriptions following specific checks (which mayinclude reviewing blood results, checking patients INR, checking any QOF codingis accurate and up to date) and dealing with patient requests.
Prescription requests are receive by patientaccess or written requests and the volume of workload is addressed within 48hours. We encourage ElectronicPrescribing with patients. You canexpect to receive hospital discharges, hospital letters and medicationrecommendations for review and action.You will inform patients when blood tests/ BP checks/ medication reviewsare due and keep their electronic notes up to date accordingly.
Our Pharmacy Technicians currently also havetelephone consultations for patients with medication queries, and support the prescribingduty doctor with requests. You will liaise regularly with clinicians insecondary care, either initiated by yourself, GP or patient. You will initiate practice policies to ensuresafe prescribing and will have very close link with the GPs, with a dedicatedtime slot each day to discuss queries and assist with decisions over patientcare. You will be a valued member of the clinical team.

Administration

Help achieve success in ICB prescribingincentive schemes
Transfer medicines information fromother healthcare agencies onto in-house patient information systems
Communicate with local pharmacies toensure efficient & well-managed repeat prescribing system
Liaise with practice doctors and staffover prescribing issues
Liaise with ICB over prescribingpolicies, targets and general issues
Liaise with nursing and residentialhomes in the practice area re medication requirements; provide andmonitor their scripts.
Liaise with patients re prescribingqueries and issues
Policy and service development
Liaise with ICB re local prescribingpolicies
Define and create protocols andformularies within the practice in consultation with other members ofthe team including the ICB medicine management team
Leadership
Working as a clinical co-ordinator toensure success in gaining Quality Outcome Framework points
Working autonomously with due attentionto agreed protocols and practice

Undertaking face-to-face medicationreviews when required
