Health Liaison Worker
We have a brand new exciting opportunity to join our PCLN Team working with adults with learning disabilities.
The Health Liaison Worker works as part of the Primary Care Liaison (PCLN) Team and supports the wider Liaison Teams to identify patients eligible for an annual health check and collate information to enable access to the wider IDD Services via the IDD SPA.
The role supports the PCLN Team in engaging service users in accessing primary care services to improve their physical health, including contacting patients and families to encourage attendance at health appointments and screening appointments.
The Health Liaison Worker also supports administration around screening projects, verification processes, and shares information with IDD SPA colleagues.
Main duties of the job
* Undertake delegated tasks to support the IDD Liaison Teams and SPA (single point of access to IDD Services), e.g. attend the SPA MDT meetings to confirm eligibility for IDD services.
* Have good working knowledge of SystmOne and Rio and other patient recording systems.
* Navigate IT systems and manage patient information.
* Provide care under basic healthcare checks (use of wheelchair scales), screening processes when competent and support to service users and carers in the community setting.
* Support and promote reasonable adjustments for patients accessing primary health settings, encourage documentation of adjustments and share known adjustments within the patient record.
* Follow up patients who did not attend (DNA) or were not brought to appointments; identify barriers and support the PCLN nurses.
* Report and record the death of a person with an IDD in line with trust policy and LeDeR guidance.
Job responsibilities
* Support the PCLN Team in investigating queries in relation to eligibility for inclusion on the GP Register and share information with colleagues.
* Respond appropriately to urgent and non-routine situations as delegated.
* Participate in team meetings and contribute to the development and effectiveness of the service.
* Ensure confidentiality is always maintained.
* Work independently and as part of a team.
* Signpost patients to supportive physical health interventions accessible to the service user.
* Work with the wider MDT and provide feedback on patient care.
* Assist patients to take care of their physical health and understand the importance of good physical health in relation to their mental health.
* Under the guidance of the qualified nurse, liaise with the wider MDT and health community when advocating for individual patient needs.
* Have a good understanding and access to IT systems including patient recording in SystmOne/EMIS and the Rio system.
* Support National Screening programmes and assist the PCLN nurses with bowel screening calls and similar screening work.
* Support blood desensitisation work.
* Undertake physical health screening where appropriate (e.g. weighing using electronic wheelchair scales and basic observations) and record and report back to the qualified nurse.
* Assist with collating patient feedback and letters to patients.
* Undertake any other reasonable duties as requested by the Team Leader.
* Work across different bases across the city and county of Nottinghamshire and be based within Clifton Cornerstone.
* Perform lone working and visit patients within their own home environments and other community settings.
* Work under the direction of the qualified nurse.
Additional information
* Post does not meet the pay level required for a Skilled Worker visa.
* Successful applicants with no prior NHS experience will normally be placed at the bottom of the band in line with Agenda for Change. Salary below minimum for sponsorship.
* Applicants requiring a Skilled Worker visa can assess the likelihood of obtaining a Certificate of Sponsorship against the relevant criteria.
* Applicants are required to pay for their DBS check; costs are deducted from salary over the first 2 months.
* Enrolment for the DBS Update Service is encouraged (annual fee £16).
* A full UK driving licence and vehicle for business use are required; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Qualifications
* NVQ Level 3 in Direct Care or equivalent experience and willingness to undertake health observations.
Experience
* Experience of working in a health setting.
* Experience of working with people with a learning disability.
* Experience of working in a health care role.
* Experience of building trust and rapport with patients.
* Experience of working in a multidisciplinary team.
Knowledge
* Understanding the role of physical health and its impact on people with learning disabilities.
Skills
* Excellent written, verbal and non‑verbal communication skills.
* Ability to prioritise tasks.
* Ability to communicate with patients/families over the telephone and in person using empathy and reassurance.
* Ability to lone work but seek advice when required and, when necessary, escalated to a senior person in a timely manner.
Training
* Evidence of training related to the role.

Disclosure and Barring Service Check

Salary
£25,760 to £27,476 a year (pro rata for part time)
