The Community Charge Nurse will play a key leadership role in managing a team of community nurses to deliver high quality, compassionate care to patients in their own homes. The post holder will work autonomously within their professional scope, taking responsibility for all aspects of community health care delivery and the safe day‑to‑day running of the service.
Responsibilities
* Line manage a team of staff and support the management of an active caseload, overseeing staff rosters.
* Participate in triage rota, carrying delegated authority to admit, refer and discharge patients.
* Take responsibility for incident management, including reviewing incident forms and ensuring appropriate follow‑up and learning.
* Contribute to the Primary Care Network (PCN), involve the wider MDT where appropriate, and delegate tasks to ensure safe and effective care.
* Provide clinical supervision to new and existing staff, working within legal and ethical frameworks and adhering to organisational policies.
* Support quality improvement by identifying opportunities for service development within safe and appropriate boundaries.
