Job Summary
Enhance End of Life Care by providing clinical expertise and advice, working in partnership across organisational boundaries to ensure care is built around the person and their family and delivered in the person’s preferred place.
Responsibilities
* Support the delivery of clinically effective, efficient and high‑quality care to people with complex specialist palliative care needs, ensuring evidence‑based practice.
* Use specialist skills to manage a caseload of patients and their families/carers with complex needs associated with living with a life‑challenging illness in a community setting.
* Act as a specialist resource across the system using a team approach to influence patient care through education, training, research, audits and service development.
* Share joint management responsibilities for the team.
* Participate in a 7‑day rota.
* Lead, engage and empower other staff, providing formal and informal learning opportunities.
* Support team development through induction, appraisals and identification of appropriate training.
* Deliver agreed targets, plan and hold regular meetings to inform, update and promote coaching and team development.
* Carry out audits on all areas to maintain standards, including spot checks and embedding improvements.
* Utilise information technology and ensure guidelines are followed when sharing information.
* Maintain a high professional standard in accordance with the NMC Code of Professional Conduct.
* If registered as a non‑medical independent prescriber, prescribe in accordance with national guidelines within limits of competence; attend relevant meetings and maintain supervision.
* Attend and be a key member of the Martlets Quality and Safety Forum and other relevant meetings.
* Access managerial support and supervision.
* Work in accordance with Martlets policies, procedures and values.
* Contribute to policy development, governance and service improvements.
* Work flexibly, contributing to improved ways of working and the development of the service.
* Manage longer‑term initiatives and improvement projects as required.
* Engage technology and optimise ways of working to improve efficiency.
* Actively contribute to team and individual development, embracing opportunities to review the service and build resilience.
* Ensure positive representation in interactions, fostering a culture of lessons learned from adverse events and enhancing patient safety and user experience.
* Promote a culture that respects and values diversity and supports patients, visitors and staff in exercising rights and recognising diverse needs.
* Ensure practice is consistent with the Mental Capacity Act 2005.
* Identify and report patterns of discrimination that undermine quality and diversity.
Qualifications
* Registered Nurse.
* Degree in a subject relevant to palliative care or equivalent specialist experience in community settings.
* Completed Advanced Physical Assessment module or willingness to undertake.
* Independent Prescriber or willingness to undertake.
* Willingness to undertake further learning as appropriate.
Experience
* Advanced Communication skills training or equivalent experience dealing with bad news.
* Complex casework and collaboration with others.
* Significant post‑registration specialist palliative care experience within charity, hospice, hospital or community settings.
* Experience working in a community environment.
* Experience managing a caseload of patients independently and managing a diverse caseload.
* Management experience including staff and budgets.
* Experience leading service development projects.
* Experience providing multi‑professional education.
Additional Information
Salary: £48,751 to £54,661 per year. Contract: Permanent. Working pattern: Full‑time, Part‑time, Flexible working.
