Overview
An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the expanding Supportive Care Team within Cardiff and Vale. The award‑winning Supportive Care service is a relatively new service development, which has significantly improved patient care and experience in advanced heart failure, liver, renal and respiratory and is now expanding into other life‑limiting non‑malignant disease areas.
Responsibilities
* Develop skills and education around clinical assessment and management of symptoms for patients in hospital and community settings.
* Work closely with the multidisciplinary Supportive Care team, including clinicians, clinical nurse specialists and therapies.
* Contribute to education, research and service development to improve care for patients in their last 1‑2 years of life.
* Engage in difficult conversations with patients and families and communicate with hospital and community teams across Cardiff and Vale UHB and other agencies.
* Develop and deliver a seamless pathway of patient care from point of referral to discharge.
* Ensure clear communication and documentation in keeping with Health Board Policy and confidentiality.
* Promote effectiveness within the team by demonstrating Health Board values, supporting staff well‑being and professional curiosity.
* Develop management plans for appropriate symptom management or support that can be offered.
* Evaluate care and treatment outcomes.
* Provide clinical advice to all members of the multidisciplinary team, patients, carers and other agencies.
Qualifications
* Excellent interpersonal and communication skills.
* Experience or enthusiasm for patient care and experience, passion, drive, commitment and enthusiasm to compassionately deliver and support excellent care for patients with life‑limiting conditions.
* Welsh language skills desirable; Welsh and/or English speakers are equally welcome.
