We are now looking for a warm, enthusiastic, and dedicated individual to join our award‑winning team as a Team Leader at Acer Court Care Home in Nottinghamshire.
Responsibilities
* Lead the person‑centred care planning process by implementing, monitoring, and conducting monthly evaluations, completing care plan audits, developing action plans, and ensuring action plans are completed.
* Assist in the smooth transition of any discharges from the home, including documentation, medication, and ensuring residents’ next of kin are fully informed where appropriate.
* Monitor the well‑being of each resident, being aware of any changes impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
* Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors and staff.
Requirements
* DBS Disclosure check – the cost of which will be met by Avery Healthcare.
* Proof of eligibility to work in the UK.
Application Criteria
* Have an NVQ 3 or equivalent in care for care homes.
* Two years’ experience working in a senior position with the resident group.
* Have the ability to communicate effectively both verbally and in writing.
* Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
