Hays are seeking a dedicated and passionate Band 6 Social Worker to join our dynamic team in Oldham on a temporary basis. The successful candidate will be responsible for providing high-quality care and support to service users and their families.
Your new role
The Social Worker will have a key role to play in relation to supporting the delivery both the personalisation and social inclusion agendas ensuring quality public facing services including health and social care services. The role will involve assessing; accessing; and implementing a range of social care services and providing guidance/advice and information for adults with acute or long-term physical health, mental health needs and for older people and carers.
Key Responsibilities
1. You would be required to assess patients under Section 117 aftercare Under the Mental Health Act 1983 using the mosaic system
2. Carry out a range of assessments including holistic “needs” led assessments, care and support planning and reviews, and, where appropriate, initiate a multi-disciplinary assessment.
3. To provide social work support to individual adults and their carers and families and involve them in all planning and actions taken, helping to signpost/refer on (where appropriate) and/or develop motivation and daily living skills.
4. To undertake assessments in response to Care Act, Mental Capacity and any legislative statutory requirements.
5. Tomunicate with a wide range of people including adults and carers, members of multi-disciplinary teams, and staff in external agencies including Primary and Secondary Health Care/Mental Health services using a variety of methods including Information andputer Technology.
6. To undertake other social work responsibilities dependent on service and individual and carer need.
What you'll need to succeed
7. ESSENTIAL - ability to assess patients under Section 117 aftercare Under the Mental Health Act 1983 using the mosaic system
8. Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA
9. Ensuring current registration with the Professional Social Work regulatory body at all times

11. Experience of working with Adults and older Adults with mental health needs and their carers
