Cares Team on a 12-month contract. In this role, you'll be the key housing specialist, providing advice, signposting, and practical solutions to support patients and residents. You'll play a vital part in reducing hospital delays, preventing homelessness, and helping people return home safely and sustainably.
What you'll be doing:
1. Acting as the single point of contact for all housing-related issues within Contact Cares.
2. Working with hospitals, support centres, and local partners to reduce re-admissions and delayed discharges.
3. Supporting patients and families by finding practical housing solutions, including re-housing, repairs, adaptations, and access to assistive technology.
4. Providing clear advice and signposting on housing services, welfare benefits, and support available across Torus, local authorities, and the wider voluntary sector.
5. Collaborating closely with colleagues across health, housing, and social care to develop new initiatives and improve outcomes for service users.
6. Recording outcomes to demonstrate the impact and value of the Housing Coordinator role.

* Evidence of working in the housing sector
* Proven experience managing a caseload and collaborating effectively with multiple agencies.
* Skilled in crisis management, risk mitigation, and making sound decisions in challenging situations.
* Empathy, excellent communication, and the ability to work with patients, families, and professionals in sensitive situations.
* A proactive and person-centred approach to finding solutions.
* The ability to work independently and as part of a multidisciplinary team.
