We are recruiting for a Patient Pathway Coordinator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
The Pathway Coordinator is the point of contact for all administrative issues relating to patients’ pathway of care from referral to treatment management and discharge.
The Pathway Coordinator works as part of a multi-disciplinary team to ensure the entire patient pathway is managed smoothly.
The post holder will work alongside existing ADHD Nurse admin to manage the ADHD service and database.

Main duties of the job
The post holder will provide professional and efficient coordination of the ADHD pathway from point of referral to diagnosis and transition to ongoing care within the service.
Oversee the delivery and assist in the development of our ADHD Diagnostic service at ENHT.
The coordinator is responsible responsibility for daily operations of the service including and identifying, meeting and monitoring quality standards of service delivery.
To participate as part of a multi-disciplinary team in the referral, assessment, admission and ongoing review of needs of the service user.
