Job Purpose
The Care Manager will provide a comprehensive, intensive community care service to individuals, their families and carers as part of an integrated, multi-disciplinary team. This service includes crisis/urgent, short-term, or planned longer‑term intervention with individuals who may also have complex health and social care needs, actively promoting their involvement and understanding of assessment processes.

Location: Aberdeen Royal Infirmary, Foresterhill, Aberdeen
Duration: Permanent, Full Time
Responsibilities
* Act as a designated lead professional in assessing and implementing inter‑agency plans when required
* Manage risk and handle competing demands while ensuring high quality, person‑centred care
* Build relationships and communicate effectively with service users, staff, carers and internal/external professionals
* Conduct interviews, investigations and assessments (including finance aspects)
* Plan, decide and review care, supporting older or disabled adults and vulnerable groups
* Write accurate reports, keep timely records and commission appropriate care packages
* Chair and participate in multidisciplinary care conferences, ensuring continuity of care
* Promote relevant services and resources, ensuring eligibility criteria are met and outcomes achieved
Qualifications & Experience
* Professional qualification in Social Work, OT, or nursing (RGN 1st level registration with community experience)
* Registration with a relevant professional body – SSSC, NMC or BHPC
* PVG membership where required (cost covered by Council for roles £30,000 and above)
* Demonstrated ability to work within a multi‑disciplinary team and manage professional responsibilities
