Kent – Qualified Social Worker – Children with Disabilities - £38
Manage a caseload of disabled children and young people aged 0-25 to assess, plan, monitor and review the support/personal assistance needs of service users, working in a person-centred way. Work with the service users and their families/carers to develop a package of support/assistance as they prepare to become adults and in their early adult life that promotes and maintains independence and wellbeing; where appropriate involve other agencies and use community resources to provide equality of opportunity, full participation and inclusion in society. The post holder should be working in line with the Social Care Capability Framework.
Responsibilities
* Caseload management for disabled children and young people aged 0-25.
* Assess, plan, monitor and review support and personal assistance needs.
* Work in a person-centred way and involve families/carers in developing support packages.
* Support transition to adulthood, promoting independence and wellbeing.
* Collaborate with other agencies and use community resources to promote equality of opportunity and inclusion.
* Operate in line with the Social Care Capability Framework.

Essential Requirements
* Degree/Masters in Social Work and SWE registration.
* 2+ years of post-qualification experience.
* Strong knowledge of UK social care legislation and practice.
* Right to work in UK.
