General Practitioner with Extended Role (GPwER) - Frailty
Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes.
The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England's website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS.
The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull. Our workstreams include: -
o Provision of comprehensive Geriatric Assessments from both within the Integrated Care Centre and Residential Care Homes located across Hull and East Riding
o Medical support to several community bedded units, used as a step up/down facility to support hospital admission prevents and the discharge to assess model (D2A)
o Advice and Guidance line accessible to Healthcare Professionals involved in the care of a frail patient including Care home staff.
o Urgent Community 2-hour Response
o Virtual Ward supporting patient with acute need to remain in their preferred place of care.
Main duties of the job
This is an amazing and varied opportunity for a general practitioner both experienced and passionate about caring for older and frail patients, who also has the drive to help further develop a high profile service care within the community. In this role you will work across all work-streams supported by the JB-CFS; assessing and pro-actively planning the care and treatment of older people and those with frailty, working closely with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support.
The ability to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, as is working experience/knowledge of local pathways and referral procedures.

Person Specification
Qualifications
* Full and current unrestricted GMC (UK) registration on the GP Register
* A teaching qualification e.g post graduate diploma in post graduate education
* A post graduate qualifica?on in older peoples care, e.g. Diploma Geriatric Medicine (or willing to work towards
Experience
* Post GMC registration training in care of older people
* 3 Years or more post GP specialist registration experience
* Experience of supervising clinical staff to ensure clinical standards are met.
* Experience of continuing professional development.
* Evidence of attendance at courses or self directed learning to meet learning gaps identified through the professional development plan and through annual appraisal
* Experience of conducting clinical audit
* Experience of designing, organising, teaching and training healthcare professionals and teaching patients
* Up to date knowledge of all the latest guidelines and evidence in older peoples medicine
* Understand the importance of evidence based practice and clinical effectiveness
* Understanding of clinical governance
* Sound clinical knowledge.
* Ability to use the evidence base and clinical audit to support decision-making
* Experience of SystmOne, Emis and Lorenzo clinical systems
