The Community Rehabilitation Team (CRT) West supports people living with complex mental health problems to achieve their goals and gain skills and confidence to live independently. CRT West works as an MDT, discussing, formulating and delivering individualised plans. We are recruiting an experienced Social Worker (Level 3) to the team on a full‑time basis.
About the Role
The role sits within our mental health complex care services in the Dorset Council area, including Community Rehab Team at Forston Clinic, Assertive Outreach Team in Weymouth and the Glendinning Unit in Dorchester. This is a new and exciting role working in an integrated way across the rehabilitation pathway. You will need to work creatively to deliver a high standard of care that optimises a person’s own health, wellbeing and recovery, providing stability and helping them develop meaningful occupation while managing symptoms of mental illness.
The social worker will lead on all aspects of social care and requires experience or keen interest in a specialist service. Knowledge and application of the Care Act, Mental Capacity Act and Mental Health Act is essential. Access to a vehicle is essential as CRT West covers the footprint of the Dorset Council Locality.
Responsibilities
Lead on all aspects of social care within the MDT, formulate and deliver individualised plans, and support clients in achieving their goals. Provide responsive care that promotes stability and recovery, integrate services across the rehabilitation pathway, and maintain up‑to‑date knowledge of relevant legislation.
Qualifications
* Degree in social work, occupational therapy or equivalent
* Registered with the Health and Care Professions Council (HCPC)
* Ability to fulfil travel requirements of the post
