Social Worker required for work within BANES in Hospital Discharge Team. 37 hours a week with rates up to £#removed#ph for experienced candidates. Minimum 3‑month contract. Flexible, hybrid working – 2 days in office.
Bath & North East Somerset Council is seeking an experienced Social Worker to join their Hospital Discharge Team. This is a fast‑paced and rewarding role focused on supporting safe and timely discharges from hospital, ensuring individuals receive the right care and support in the community.
Key Responsibilities
* Support hospital discharges in line with discharge to assess pathways.
* Complete Care Act assessments and develop person‑centred care plans.
* Work collaboratively with health professionals, families, and community services.
* Undertake safeguarding enquiries where required.
* Promote independence and ensure positive outcomes for service users.
* Manage a caseload effectively within a busy hospital setting.
Qualifications
You will be SWE registered, ideally a driver with access to own car and able to reside and work in the UK.
