Key Responsibilities 1.Works under indirect supervision of the Practice Nurses and Nursing Lead 2.Is accountable to the Nursing Lead and the Primary Care Network for their actions in undertaking delegated work. 3.You will be responsible for attending to patients who wish to be seen by a health care professional within the Patients Home, providing high quality evidence-based nursing services to the population of Shore Medical. 4.Provides support to other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole. 5.
Refers to more senior members of the Primary Health Care team as appropriate. Clinical Responsibilities The Health Care Assistant undertakes procedures that are agreed by the Head of Nursing which are likely to include: 1. Venepuncture 2. BP checks, height, weight, BMI, urinalysis, ECG 3.
NHS health checks 4. Long term condition/Chronic disease health checks 5. Flu & Covid vaccination (following approved training and competence checks 6. Diabetic Foot Checks (following approved training and competence checks) 7.
