Specialist Community and Community Diabetes Dietitian
We are looking for an enthusiastic Band 6 Dietitian to join our friendly team to work as a key, valued and active member of the Community Dietitians team based at St Richards Hospital, Chichester.
This is permanent part-time post. The applicant must be self-motivated, well-organised and be confident communicator.
Informal visits or discussions about the role are very welcome.
Main duties of the job
The post holder will work for half of their time alongside the MDT community diabetes team seeing patients in out-patient clinics and providing patient information in GP clinics across the area.
The other half of the post will be providing dietetic advice to patients at home or in community hospital settings.
This post offers excellent opportunities to develop working relationships with a wide MDT network including GP's Diabetes Specialist nurses, Practice nursesand Community-based Healthcare professionals.
The post holder will participate in continuing professional development, audit and research within this specialist area to further best practice and personal development.
They will also be involved in supporting junior dietitians and students.
Regular travel between sites is required, a car driver and access to a car for work is essential.

Job responsibilities
* Undertake dietetic assessments of referred patients taking into account medical, social, cultural and psychological factors. Calculate nutritional requirements based on the interpretation of anthropometry, biochemistry, clinical condition and other physical parameters.
* Determine and prescribe the appropriate dietetic treatment and develop treatment plans.
* Provide training to patients, carers and staff system and order equipment/appropriate ancillaries patients.
* Negotiate change with patients to enable them to achieve targets, overcoming barriers to change including dietary, environmental, social and psychological factors.
* Communicate complex information in an understandable form to patients who may have barriers to communication and understanding.

* Bachelor of science (Honors degree) Nutrition and Dietetics, Diploma in Nutrition and dietetics, or post graduate Diploma

* Post registration training in cognitive behavioural therapy.
* Student clinical facilitator training course

* RELEVANT POST GRAD COURSES
