We have an exciting opportunity for the post of a Personal Advisor in our Care Leaving Team, as part of our Outstanding Social Care Workforce. If you are driven, flexible, highly motivated and passionate about improving outcomes for our care leavers of North Lincolnshire then this is the job for you.
You will be joining a strong and supportive team who work with young people aged 16+ who are Eligible, Relevant, Former Relevant and Qualifying Care Leavers offering support and guidance to enable young people to make a successful transition into adulthood.
You will hold a case load of care leavers providing 1:1 support and undertake the role of a Personal Advisor as described in the Children (Leaving Care) Act 2000 and the Children and Social Work Act 2017.
This includes the preparation and coordination of pathway planning alongside young people. The Leaving Care Team is committed to ensuring young people are prepared and supported to live independently, have improved access to education, employment and training, experience stability in their lives, feel safe and secure, receive improved access to health support and achieve financial stability.
To apply for this post you will have experience of working with young people in a professional capacity and have an understanding of the challenges facing care leavers as they make the transition to independence. You will also have enthusiasm and creativity in developing meaningful relationships with young people. You will be ambitious for their future and support them in accessing education, employment and training.
