Job Purpose
* Checks for safety, accuracy, appropriateness and completeness with respect to drug, dose, route, frequency with consideration given to patient-specific data including laboratory values, age, gender, height, weight and BSA.
* Prepares controlled substances to be dispensed, completing the appropriate documentation while maintaining an accurate perpetual inventory.
* Performs weekly inventory and reconciliation of controlled substances. Notifies Manager, Ambulatory Pharmacy Services or Senior Director of Pharmacy of any unexplained discrepancies.
* Performs daily oversight of all Pharmacy Technician activities; checks/reviews all Pharmacy Technician work according to regulatory requirements.
* Dispenses medication to ambulatory patients with appropriate medication counseling.
* Educates/trains patients to administer and monitor therapy as required.
* Effectively interacts with prescribers concerning drug order clarification or drug therapy modification.
* Ability to perform without direct professional supervision and to oversee the work of Technicians and students.
Qualifications/Requirements
Education/Training: Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required.
Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to working 1000 hours at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA.
Work Experience: Minimum of three (3) years experience as a pharmacist is preferred.
